To foster a stronger working relationship and ensure we can leverage your expertise effectively, we kindly request you to complete and submit the following forms. These forms will help us gain a deeper understanding of your business, the products and services you offer, and your organization's capabilities.

1. Vendor registration form online available in this link (Online Vendor Registration) or via offline form in the links below. 

2. IRS Form W-9 (Request for Taxpayer ID & Certification)

3. Product/service  brochures & similar literature

4. Other documents that may be required in accordance to requirements of a bid or proposal that G4S will be participating on (e.g. Business License, Certificate of Insurance, Financial Statements, et.al.)

If completing vendor registration form online, please also submit items 2 and 3, otherwise, submit all items 1 thru 3 to Procurement Dept  (purchasing@gu.g4s.com) or Fax at +1 (671) 649-7245.
